Please enter as much information as you can, because the more we know about you, the better we can treat you. Also by preregistering it speeds up your first visit with us. Once you have finished your initial registration, you can use the features of myHealthManager. myHealthManager empowers patients to take an active part in managing and improving their health. As you contribute to your online journals in myHealthManager—tracking weight loss, nutritional intake, exercise, blood sugar levels or other variables, we can access and track your progress.
